4-H Summer Camp Camperships
Provided By the 2012 ROBERT P. AMMAN MEMORIAL CAMPERSHIP FUND
If you wish to be considered for a campership to send your child to 4-H Summer Camp, please fill in this form COMPLETELY and send to Dennis DeYoung / 9401 Reseda Blvd., Suite #100/ Northridge, CA 91324. Applications must be postmarked NO LATER THAN FRIDAY, JUNE 1, 2012. You must have already submitted a Registration Form and paid the camp reservation fee of $75.00. If you fax or email the application, please follow up by mailing the application to the address below. Failure to complete this form accurately and completely may delay or negate your request.

Camperships
Financial assistance is available to boys and girls aged 8 to 19 on an individual, case by case basis. Assistance is based on need and fund availability. All information for financial assistance is kept confidential. Camperships are not guaranteed and may be issued in any dollar amount up to the maximum of $200.00.
The Campership application is on the reverse side of this page. The parent/guardian must also submit a completed camp registration form, medical release form, other forms and the $75.00 deposit directly to Keith Nathaniel at the L.A. County 4-H Office (available at www.4Hsummercamp.com). If you do not register for camp, you cannot attend (i.e., no walk-ins). CAMPERSHIP APPLICATIONS MUST BE POSTMARKED NO LATER THAN May 1, 2012 for the Early-Bird Registration. Camp Registration Fees are: $275 if application sent prior to 5/1/2012; $350 from 5/1 to 6/1 and $400 after June 1st. The Memorial Campership Fund Selection Committee will send Campership vouchers to the address on this application by June 11, 2012. No cash or monies will be awarded directly to campership recipients. The Robert P. Amman Memorial Campership Fund will issue a check with a list of recipients directly to Mary Lash, Treasurer, L.A. County 4-H Club. Chaperones, Resource Leaders and other 4-H volunteers who are 21 and older do not have to pay the reservation or registration fee, but will have to complete a Registration Form, Medical Release Form and other forms required to attend Camp.
The Robert P. Amman Memorial Campership Fund was organized in 1986 after the untimely death of Robert Amman, an avid 4-H Summer Camp participant and Head Youth Director (1981). Over the past 20+ years there have been 613 partial and full camperships awarded totaling $101,276. The Memorial Campership Fund is an IRS Section 501c(3) non-profit organization and tax-deductible contributions are encouraged. Please contact us if you (or someone you know) would consider making a tax-deductible contribution to fund future camperships. In 2002, contributions from the Code Memorial Fund were merged into the RPA Memorial Campership Fund.
Campership Policy
Camperships are available to assist families who may not otherwise be able to finance the fee to attend the one-week 4-H Summer Camp program which is operated by the Los Angeles 4-H Youth Program. A Selection Committee, appointed by the President of the RPA Memorial Campership Fund and/or his designee, will process all completed forms and notify the parent/guardian of the amount awarded to each applicant no later than June 11, 2012 4-H Summer Camp Camp Committee Chair Dennis DeYoung at (818) 280-0280 at www.campership.com to the maximum of $200.00. Awards will be made based on the financial need of the child and the availability of funds. The committee, in allocating financial assistance will consider the following criteria:
1. Applications for a Campership must be postmarked by June 1, 2012. Late applications will not be considered.
2. Applicants must be enrolled members of 4-H as of the first day of Camp. Contact the County Office for more information – applicant does not have to be registered in a local 4-H Club, just enrolled in the program.
3. Preference may be given to children who have not received camperships in prior years.
4. Volunteer Camp Staff are eligible to receive camperships and may also be given preference in awarding camperships.
5. Camperships will be awarded regardless of where an applicant resides, but will only be awarded to attend the 4-H Summer Camp, June 1 – July 7, 2012 at Camp Seeley in Crestline, CA.
6. Camperships are not guaranteed and may not be available in any given year. Camperships are not awarded on a first-come, first-served basis. There are no refunds from Camp or the Campership Fund if your child does not attend camp or goes home early for whatever reason.
